Important Health Requirements for 2017-2018

Students entering Early Childhood, Kindergarten, Sixth and Ninth grades are required to have a current physical examination completed on the approved Illinois Department of Public Heath Form.  A sport physical does not meet the requirement.   A current physical is one that was completed within one year of August 2017.  The parent portion on the upper back side of the form must be completed and signed.  Students entering grade 12 in the State of Illinois must be immunized for Meningitis.  The immunization requirement consists of two doses of the MCV4 vaccine for entry into 12th grade.  Only one dose of the MCV4 is required for students entering 12th grade if the first dose was received on/after the age of 16.

Immunization records can be viewed through the Parent Infinite Campus Portal.  Physical examination forms are available in all School Health Offices and on the D300 Health Services website.

D300 together with Greater Elgin Family Care Center offers physical examination clinics throughout the year.  Please visit the D300 Health Services website or contact any D300 School Health Office for dates and locations.
